1. Proof of Illness / Medical Certificate

  • A signed medical certificate from your doctor confirming:
    • You’re unable to work due to medical reasons.
    • The expected duration of your recovery.
  • Having an open hysterectomy, your doctor will likely estimate 6–8 weeks off.
    • Some doctors charge for this certificate — ask in advance! (wtf)

2. Record of Employment (ROE)

  • Your employer must issue a Record of Employment once you stop working.
  • It can be sent:
    • Electronically (to Service Canada).
    • Or given to you as a paper copy.
  • Check with HR to make sure it’s submitted promptly.

Make sure you have ready:

  • SIN (Social Insurance Number)
  • Full address and contact info
  • Dates of employment (start and expected leave date)
    Details of all employers in the last 52 weeks
  • Total hours worked in the last 52 weeks
  • Your reason for leave (medical)

5. Banking Info

  • Direct deposit form from Bank

6. Apply for EI Sickness Benefits

  • Do this as soon as you stop working, even if your ROE isn’t ready yet.
