1. Proof of Illness / Medical Certificate
- A signed medical certificate from your doctor confirming:
- You’re unable to work due to medical reasons.
- The expected duration of your recovery.
- Having an open hysterectomy, your doctor will likely estimate 6–8 weeks off.
- Some doctors charge for this certificate — ask in advance! (wtf)
2. Record of Employment (ROE)
- Your employer must issue a Record of Employment once you stop working.
- It can be sent:
- Electronically (to Service Canada).
- Or given to you as a paper copy.
- Check with HR to make sure it’s submitted promptly.
3. My Service Canada Account
- If you don’t already have one, create it here: My Service Canada Account (MSCA)
- You’ll need this to apply and monitor your claim.
4. Personal Info & Work History
Make sure you have ready:
- SIN (Social Insurance Number)
- Full address and contact info
- Dates of employment (start and expected leave date)
Details of all employers in the last 52 weeks - Total hours worked in the last 52 weeks
- Your reason for leave (medical)
5. Banking Info
- Direct deposit form from Bank
6. Apply for EI Sickness Benefits
- Do this as soon as you stop working, even if your ROE isn’t ready yet.
